A wedding at one of the resorts, a long dinner, a beach club afternoon that went on. A hangover at thirty degrees is a different proposition from one at home, because dehydration is coming at you from several directions at once. Hangover treatment in Mai Khao is fluids, salt and time — occasionally delivered through a needle when the clock matters.
Why it hits harder here
Alcohol is a diuretic, so you are losing fluid while drinking it. Then you sweat through the night in a hot room, sleep badly, and drank less water than usual because you were drinking something else.
Most people also start the evening already down on fluid after a day in the sun, so the deficit is deeper than the drinking alone accounts for.

What genuinely helps
Fluid with electrolytes in it. Rehydration sachets from any pharmacy here outperform anything sold at four times the price, and coconut water is a decent local stand-in.
Get something inside you, preferably carbohydrate with a bit of salt on it. Sugar levels tend to have dropped overnight, and a virtuous breakfast of fruit and vegetables is the wrong response to that particular morning.
Paracetamol for the head rather than ibuprofen, which is harsher on a stomach that has had a night of alcohol. If you drink heavily and regularly, say so — paracetamol is not the right choice in that situation.
What does not
Another drink. It postpones the reckoning and deepens the dehydration you are already carrying.
A punishing gym session or an hour in the midday sun, both of which increase fluid loss at precisely the wrong moment.
And a fourth strong coffee. One is fine; several add a mild diuretic effect and make the shakiness and anxiety worse.
When a drip earns its place
Genuinely, when nothing will stay down. If everything you drink comes straight back, oral rehydration has stopped being available as an option and intravenous fluid goes around the problem.
Deadlines are the second justification. When a plane leaves at two, or the ceremony starts at four, an hour on a drip buys back a day you were otherwise going to lose lying down.

Salt water, essentially — plain saline or one of the balanced solutions — and that is the ingredient earning its keep. Alongside it, something to settle the stomach where sickness is the problem, plus analgesia for the head.
Vitamins are commonly added and are harmless, though the improvement people attribute to the B vitamins is really the litre of fluid.
Budget an hour of your morning, wherever you are staying. Nobody should mind being asked to run through the contents of the bag first, and you are within your rights to want that.
When it is not a hangover at all
This is the part that matters. Fever, a stiff neck, a headache unlike your usual ones, confusion, or symptoms worsening rather than easing through the afternoon — none of that belongs to a hangover.

Retching that carries on well past the morning, anything bloodstained coming up, or a gripping pain in the abdomen all sit outside what a heavy night explains.
Blood may be bright red or look like coffee grounds, and either needs assessing the same day rather than being written off as a rough night.

The two things that actually kill people here

The pattern worth naming is the one where somebody who would never ride home drunk at home does it here, because the distances feel short and the roads feel empty. Two kilometres back along an unlit coast road at one in the morning is not a short trip on a machine you have ridden for four days, and the taxi that would have covered it costs less than a round of drinks.
Making the next one less bad
Water alongside rather than afterwards. A glass between drinks does more than a litre at three in the morning, because absorption needs time you no longer have by then.
Have a proper meal beforehand and keep picking through the evening. A full stomach slows the alcohol reaching your bloodstream by a measurable amount, and the nights that end badly are almost always the ones where dinner got skipped in favour of another round.
Buy the rehydration sachets on the way back to the hotel while you still care. They are pennies in any pharmacy along this coast, and the version of you who intends to pick some up in the morning has never once followed through.
