Sprained ankle treatment in Mai Khao, Phuket

Soft sand, a wet pool surround, an unlit path back from dinner, or a step down from a villa terrace. Sprained ankle treatment in Mai Khao begins with the only question that matters in the first hour — whether anything is broken — and then with getting you walking again rather than losing the week.

Broken or sprained

Doctors use a standard checklist here, and it predicts fractures far better than either the level of pain or the size of the swelling does — both of which mislead constantly.

Imaging is justified in three circumstances: the ankle will not carry you for four paces; pressing along the rear border or the very tip of either malleolus produces sharp bony pain; or there is that same pinpoint bone pain over the navicular or at the base of the fifth metatarsal further forward in the foot.

The opening two days

Wrap it snugly in a stretch bandage, stopping well short of anything that leaves the toes pins-and-needly or pale. Then get the foot up higher than your hip every time you sit down. Ankles swell more in this climate than they would in a European autumn, which makes the propping-up part matter more here than the textbooks suggest.

What sets it back

Steer clear of warmth on it, hot water aimed at it in the shower, rubbing it, and drinking, for the opening forty-eight hours. Every one of those opens the blood vessels up and puts more fluid into an ankle that is already puffy.

Walking it off on the beach. Soft uneven sand is among the worst surfaces for a recently injured ankle, and re-injury on exactly that ground is common.

Also resist binding it firmly before bed. A bandage that goes right round a joint which is still filling up can quietly cut off the circulation for hours while nobody is awake to feel it going wrong.

Grades, and honest timescales

Stretched ligaments — mild swelling, walking possible — recover in one to two weeks.

A partial tear, with significant swelling and bruising and painful walking, takes three to six weeks.

A complete tear with real instability takes months. Most holiday ankles are the first two, and the common error is expecting the second to behave like the first.

The bruise that appears somewhere else

Around the third day a startling patch of purple tends to surface in the sole or spread down among the toes — nowhere near the ligament that actually tore.

That is gravity moving blood downward through the tissues, not a new injury and not a sign that something was missed.

It looks far worse than it is. What matters is whether pain and function are improving, not what colour the foot has gone — see cuts and lacerations.

Pain relief that helps

Ibuprofen and its relatives do real work on both the ache and the puffiness; take them after eating. Paracetamol layers on top of that quite happily rather than replacing it.

Rub-on gel of the same family reaches a joint sitting this near the skin surface perfectly well, and there is no reason not to use it alongside the tablets.

Avoid anti-inflammatories with stomach ulcers or kidney problems, and — importantly here — with any suspicion of dengue, where they affect bleeding risk — see dengue fever.

The injury that gets missed

There is a variant that tears the tissue binding the tibia and fibula together, up above the joint line, instead of the familiar band on the outer side. The soreness sits higher than expected, and it comes from the foot being wrenched round rather than simply going over.

Recovery runs much longer and the handling is not the same, which is why this variant turns up so often among people still limping eight weeks on.

Pain sitting higher than expected is worth mentioning specifically rather than assuming it is all the same injury.

Getting it working again

As soon as the acute stage eases, get it moving. Tracing letters in the air with your big toe does the job as well as any prescribed routine.

After that, practise balancing. Half a minute on the bad leg alone, working up to doing it with your eyes closed. Few people bother, and they should: the nerve endings telling your brain where the joint is in space get torn along with the ligament, and they need retraining separately.

Skipping that is why one sprain so often leads to another. Strength returns before balance does, and the gap between the two is where the next injury happens.

The rest of the holiday

Get in the pool once any broken skin has sealed — it is actively good for it, since your body weight comes off the joint while the ankle still gets to work through its range.
